     Surgery

Unfortunately or fortunately, surgery remains an important treatment method of the modern dentistry. Tooth extraction indications are different: complete destruction of a tooth crown when its full restoration is impossible, tooth fractures, very loose teeth and, in some cases, wrongly located teeth are removed. Teeth are also removed because of orthodontic indications. Most frequently, these are third molars which may have wrong location or don’t erupt at all. Pressing at other teeth, they make them shift and cause disturbed occlusion.

Wrong eruption of wisdom teeth leads to an inflammatory process. In our clinic, we perform all manipulations using modern anesthetics (Ultrakain, Septanest, Mepivacain) quite painless. We also use the Injex method of needleless anesthesia. You have no reason to worry even about tooth extraction. Moreover, our surgeons continuously improve their qualification. Don’t despond, periodontitis (granuloma, cyst) do not necessarily mean that your tooth must be extracted. Specialists working at our clinic can offer you alternative methods. Tooth root top resection is an operation allowing to keep the tooth. At the same time the root top will be cut off together with a pathological focus (cyst, granuloma).

This operation can be carried out in the front and lateral teeth areas. In case it is not possible to keep one of the roots of the lateral teeth, another operation can be carried out. Its name is hemisection. This surgery is meant to extract bad root followed by use of a healthy remained one for prosthesis.

Tooth-keeping operations.

Don’t hasten to extract a tooth not allowing any therapeutic treatment, since there exist several operations which can keep your tooth for many years:

  • tooth root top resection – extraction of a tooth root part with the chronic infection focus (cyst, granuloma)
  • hemisection, i.e. cutting off the affected root of a multi-root tooth which allows saving some part of the crown and remaining roots for further prosthesis. This will be a more reliable support than an implant.

Tooth Extraction

If it still necessary to extract the tooth, we suggest to perform it painlessly, with a minimal injury of surrounding tissues, with usage of modern anesthetics, antimicrobial and osteogenic preparations which allows to escape future post-operative complications.

For Your Information

  • Anesthesia – is carried out in several steps: application (spray or gel is used), and only after that an anesthetic is injected, practically without any feeling of discomfort.
  • Modern qualitative dentist equipment allows to perform extraction faster and more neatly which stimulates faster regeneration.
  • Non-traumatic extraction means that in the course of extraction, it is essential not only to pull the tooth out, but to keep as much of tissues located around this tooth as possible.

Tooth Extraction Indications

Tooth extraction is to be carried out only in the worst case, when it is not possible to save it any more, or when such a tooth can be the cause of other, more harmful complications. For example, when there are teeth which can cause cysts, inflammations or trigeminal neuritis. If the tooth constantly injures the tongue or the buccal mucosa, or hinders the normal occlusion, it is better to extract it. The most frequent indication for tooth extraction is necessity of mouth cavity sanitation together with chronic periodontitis at the acute stage when elimination of the inflammatory focus near the tooth top will not work. Multi-root teeth are to be removed, too, for they could bring to odontogenic osteomyelitis.

A dentist can extract teeth when a removable denture is being adjusted, but for that it must have strict indications. Tooth extraction is a surgical operation. In the course of this operation, the tooth is pulled out of the jaw’s alveolar outgrowth lunule with special instruments. For that, it is necessary to detach the gum to destruct the ligamentous apparatus linking the tooth with the alveoli and to extract the tooth.

All these operations are performed under anesthesia. If a multi-root tooth is to be extracted, the dentist needs to determine peculiarities of the tooth’s structure, the location of its roots and their deviations. For this purpose, the tooth is studied via X-ray in different projections which allows to escape possible complications while its extraction. There are several conditions when tooth extraction is contraindicated. Tooth extractions contraindications are divided into absolute ones and relative general ones.

Tooth extraction is absolutely contraindicated under conditions of the decompensation stage of cardio-vascular system, acute myocardial infarction, as well as blood diseases with clotting system malfunction. Relative contraindications are acute viral respiratory diseases, some diseases of nervous system (psychoses, epilepsy), hypertonic disease, ischemic heart disease. In such cases, it is necessary to achieve stabilization of the patient’s state and then to extract the tooth.

Absolute local contraindications include angina, acute stomatitis, gingivitis. Before the scheduled tooth extraction operation, the dentist must inquire thoroughly if the patient has any diseases and allergic reactions. If the patient is over-anxious or nervous, then, in this case, premedication with use of sedatives must be carried out. And only after that anesthesia and tooth extraction can be started.
